Request Medical Records

The Urology Group, PC has partnered with HealthMark Group to manage medical record requests. HealthMark is a trusted partner that specializes in facilitating medical record release and keeping your personal health information safe.

How do I request my medical records? 

You can click on the button at the top to enter your email and start the process. Once your request is complete, you will receive an email with a secure link to download your records.  You can also contact HealthMark at 1-800-659-4035 to request records by phone.

How long does it take to process requests?

Records can be available for download in as few as 8 business hours! However, some records may be subject to physician approval and can take up to 5 business days. If it has been more than 5 days since you made your request, please reach out to HealthMark at status@healthmark-group.com for an update.
