About Your Visit

For your initial visit, you will be asked to provide a medical and surgical history, including all medications you are currently taking (prescribed medication as well as over the counter). You should be aware of the dosage, as well as the names. You should bring any previous medical records or test results in your care that may be helpful. Our receptionist will assist you in obtaining any x-ray or reports. They will need the name of the facility where the procedure was performed.

You will also need to bring your insurance card and information if your insurance has specific requirements. (i.e. second surgical opinion, pre-certification for hospitalization).

“No Shows” and late cancellations inconvenience those individuals who are in need of medical treatment. In order to be respectful of the needs of other patients, please call our office promptly if you need to cancel or reschedule your appointment. We require that you call at least 4 hours in advance.

Failure to give a 4 hour advance cancellation or being a “No Show” will result in a non-refundable charge of a $25.00.
