The responsibility of payment of bills rests with the patient. Today, many insurance companies require managed care including pre-authorization, second surgical opinions and limitations on hospital days. We ask you to assume responsibility for informing our patient representative if your coverage has any of these requirements. If your health care is managed by an HMO or a PPO, you may need a special referral letter.

Because insurances are constantly changing, please call your insurance office to make sure we are still affiliated with your company. Along the same line, if you do not see your insurance listed on our site, call to see if we have added your insurance to our list of affiliates.
